Mouse: Shot by SWAT in the Matrix
Dozer: Electrocuted by Cypher using lightning rifle
Apoc: Disconnected from Matrix when Cypher retrieves the stuff from the back of his neck, killing him
Switch: Disconnected from Matrix when Cypher retrieves the stuff from the back of her neck, killing her
Cypher: Electrocuted by Tank (which survived after being electrocuted by him) using lightning rifle
Neo: Shot by Smith in the Matrix. He comes back to life when Trinity kisses him.
Agent Smith: Exploded when Neo jumps into his body. Note: As a humanoid manifestation of a computer program, he returned as an improved version in the sequel.

Tank - Died in between the events of the first film and Reloaded from unspecified causes.
Bane - Assimilated by Agent Smith.
Abel - Shot in the head by Persephone.
The Keymaker - Shot to death by the Smith clones while escaping with Neo and Morpheus.
Trinity - Shot in the chest by Agent Thompson; revived by Neo.
Axel - Fell to his death during the bridge collapse.
Jax - Impaled on a broken bridge caused by the Sentinel bomb.
Ajax - Killed by the Sentinels.
Binary - Killed by the Sentinels
Soren - Killed by the Sentinels.
Vector - Killed by the Sentinels.
Ballard - Killed by the Sentinels.

Seraph - Assimilated by Agent Smith; revived in the rebooted Matrix.
Sati - Assimilated by Agent Smith; revived in the rebooted Matrix.
The Oracle - Assimilated by Agent Smith; revived in the rebooted Matrix.
Maggie - Stabbed in the stomach with a scalpel by Bane.
Mifune - Fatally wounded after being attacked by multiple Sentinels.
Charra - Impaled through the back by Sentinels.
Bane/Smith - Bludgeoned with a lead pipe by Neo.
Trinity - Impaled by multiple spikes when the Logos crashes in Machine City.
Agent Smith - Destroyed by the Deus Ex Machina once it was connected to him by Smith turning Neo into a Smith. This in turn also destroys the multiple clones Smith created.
Neo - Succumbs to injuries from the Deus Ex Machina's energy that channeled through him to destroy Smith.
