The Butcher Creek Dig Site Accident was an off-screen event that can be seen affecting the surrounding local area and is the focus point for the mission "Wisdom of the Elders" in Red Dead Redemption 2.
The event was caused when Miners working for the Roanoke Fuel Company accidentally mined out a pocket of capsian and lead, which fumed the mines with toxic gas and poisoned the Elysian Pool.
